How to Submit a DMCA Notice

To help us process your request efficiently, please include the following information in your notice:

A clear identification of the material you believe is infringing, including its location (such as a specific URL) on our website;
Your full name, mailing address, telephone number, and email address;
The following statement:
“I have a good faith belief that the disputed use of the copyrighted material is not authorized by the copyright owner, its agent, or the law (e.g., as a fair use).”
The following statement:
“The information in this notice is accurate and, under penalty of perjury, I am the copyright owner or am authorized to act on behalf of the owner of an exclusive right that is allegedly infringed.”
Proof of copyright ownership (such as legal documentation);
Your physical or electronic signature.

The subject line of your email should read: “DMCA Copyright Notice”

Misrepresentation Warning: Title 17 USC §512(f) provides penalties for knowingly submitting false claims, including liability for damages and attorney’s fees.
